In a retrospective study, we reviewed all cases of drug-induced pleural and pericardial effusions and the resulting complications over a 2-year period in a general hospital in Hong Kong. Thirty-five patients with pleural effusions and nine with pericardial effusions were identified. There was no difference between the 2 year and 4 year cumulative incidence (two year mean 8.5% vs 4 year mean 7.5%, p = 0.734). Pleural effusions were usually self-limiting (67.1%) and only one was associated with persistent symptoms. There was one case of fat embolism and 3 cases of pneumothorax. Pericardial effusions were more common in patients with dementia and with renal dysfunction. They were usually self-limiting but were complicated by pneumopericardium, tamponade and cardiac failure in 4 cases, and in one patient, left ventricular failure developed 2 months after the discontinuation of diuretics with continued signs of hypokalaemia, with only a 25% fall in serum creatine phosphokinase.
